WHAT IS YOUR NEW PATIENT PROCEDURE?
After contacting Brooke via email regarding availability, she will be happy to set up a phone call to confirm client is appropriate for physical therapy and answer any questions. A new patient evaluation will be scheduled at the location of the patient's choice. This includes assessment of subjective and objective data to identify movement dysfunctions, establish goals, determine treatment frequency, and establish home exercise program.
WILL INSURANCE COVER MY VISIT?
Seaside Physical Therapy is a private-pay model. We are considered out-of-network with insurance however, upon request I will provide an invoice with treatment documentation and corresponding medical codes for submission to insurance provider if you are eligible for out-of network reimbursement benefits.
WHY PRIVATE-PAY?
At a competitively priced flat rate, I can optimize our treatment time based on patient's specific needs, eliminating wait times in between providers to treat the impairment as soon as possible. This model actually reduces overall health costs and offers transparency with no hidden bills or unexpected copays. Many in-network, insurance covered physical therapy services have similarly priced copays. We value time effective treatment with patient education focus to restore pain free function as soon as possible.
This billing model is used to enhance and simplify the patient experience through a patient centered approach, as opposed to a generic, insurance centered approach needed to obtain reimbursement or reach clinic productivity quotas.
DO I NEED A PHYSICIAN'S REFERRAL?
Florida allows 30 days of unlimited access to a physical therapist before a doctor's referral. If patients want to continue treatment after 30 days, a referral from a MD, nurse practitioner, chiropractor, or podiatrist will be required. If the patient already has a physician's referral for physical therapy, I am happy to treat and communicate with referring physician when necessary or requested.
DO YOU TAKE MEDICARE?
Unfortunately, Seaside PT does not participate in Medicare. Due to current laws, If a non-enrolled Physical Therapist provides services to a Medicare beneficiary that would normally be covered by Medicare, he/she is required to bill Medicare directly and is not allowed to accept self-payment for these services. Therefore, we cannot provide PT services to Medicare patients. However, Medicare does allow us to accept beneficiary's out of pocket payment for a "non-covered" service including overall strength/endurance maintenance, prevention, wellness or fitness. This can be useful for fall prevention or general conditioning purposes. If you would like to use visits for these purposes, I am more than happy to treat you.
DO I NEED EQUIPMENT FOR THE APPOINTMENT?
No, a clean space for movement and a table is preferred. If there is a specific piece of exercise equipment you would like to work with, I am happy to educate and incorporate it into our treatment.
ARE VIRTUAL APPOINTMENTS AVAILABLE?
Yes. This is a great option to achieve pain relief for clients not in the Jacksonville area. Brooke will send you a zoom link for an evaluation or follow up treatment session. Treatment will include all most aspects of an in-person visit, and modifications will be made to replace hands on aspects.
